After purchasing the indicator, you will be given access to the paid download page for this indicator.
Login and go to: Store > Manage Purchases > Manage

Then click on this link to download the file.

Download the file and unzip it. That will give you an .EX4 file that you will install in Metatrader 4.

Installation
To install the indicator, click on the MetaQuotes Language Editor (MLE) button. You can also press F4.

This will open the MLE program. Click and drag the .EX4 file to the indicators folder.

Now restart Metatrader 4 for your new indicator to be identified. You can also close the MLE program.
Setup the Indicator
Once MT4 restarts, you can click on the Indicators List button to get a list of indicators. Go to Custom > TH Simple RSI Alerts. When you select it, MT4 will add the indicator to the chart and show you the setups window.

Basic Setups
Now the setup screen will pop up and you can configure your settings. When you click on the Inputs tab, these are the defaults.

Input Values Explained
RSI Period – The number of previous periods that the indicator will use to draw the graph. PopupAlert – Set to true to get a popup alert on your Metatrader screen.  It will be a small alert box. SoundAlert – Set to true to get a sound alert with your popup alert.  It is the default alert tone. EmailAlert – Set to true to get an email alert sent to the email address of your choice.   PushNotification – Set to true to get a push alert on your mobile device.  You have to have the Metatrader 4 app installed on your mobile device to receive push alerts.  OverBoughtLevel – The overbought level where you want the alert triggered. Usually set to 70 or 80. OverSoldLevel – The oversold level where you want the alert triggered. Usually set to 20 or 30. MinAlertsMinutes – You will only get one alert per time period set in this field.  For example, if this value is set to 5, you will only get one alert every 5 minutes.  The RSI value might move in and out of the overbought or oversold level multiple times, generating multiple alerts.  This could be very irritating, especially on lower time frames.  Set this to a value that makes sense to you. Important Notes
Alerts are only generated on the close of a candle. So RSI has to close at or above/below the signal levels to actually generate an alert. This eliminates false alerts. Alerts are only generated on the time frame that is displayed on the chart.  To have alerts from multiple time frames, open one chart per time frame. Push Alerts Example
When you get a push notification from the RSI indicator, this is what it will look like on your mobile device, inside the MT4 app.

Final Tips
Remember, your computer must be turned on for the alerts to work. You can either keep your computer on at home (not always reliable) or you can use a Virtual Private Server (VPS).
I recommend using a third party VPS, it's more reliable.
That's it, happy trading!
